BASIC Registration Form

The University of Wisconsin-Madison School for Workers is hosting the 2019 Basic Institute at the Madison Concourse Hotel (directions) in Madison, Wisconsin.

  • The class begins August 11, 2019.
    • Registration begins at 5:00 p.m.
    • Dinner at 6:00 p.m.
    • Orientation at 7:00 p.m.
  • The class ends August 16, 2019 at noon.

PLEASE NOTE ALL ROOMS ARE NON-SMOKING.

Fees: Single room: $1,570.00
  Double room: $1,300.00
  Extra nights: $127 - single occupancy room
$147 - double occupancy room
+ $10 parking, per night (no meals)
    (Please indicate early arrival/late departure on the registration form)

Fees include dinner Sunday evening, tuition, lodging, and parking. Breakfast, lunch, and afternoon refreshments will be included Monday – Thursday.

REGISTRATION INSTRUCTIONS:

STEP 1: Complete the registration form.

  • Be sure you have completed both Section 1 and Section 2 for each person attending.
  • Mail or fax the included form by July 12, 2019 to:

    Note: Please do not contact the Madison Concourse Hotel for room reservations.

    Completing this registration will confirm your attendance and reserve your room at the hotel.

  • You will receive payment instructions after you submit your registration form.
  • If you want to send members but cannot submit your forms by July 12, 2019, please contact Madison Everson.

STEP 2: You will receive a confirmation letter from the University of Wisconsin-Madison School for Workers indicating the details of your registration. A map will be included.
